What Is Testosterone Cream?

Have you been skeptical of testosterone creams? Maybe they’ve never been your top choice, or you’ve assumed they just don’t work. And honestly, you wouldn’t have been wrong.

Older transdermal creams had low concentrations and poor absorption, especially when applied to areas like the arms or shoulders. Between sweat glands and hard-to-control dosing, they often left men underwhelmed.

That said, transdermal testosterone has been around for years—and it’s come a long way. Today, when testosterone is prescribed as a 20% compounded cream in the right base (formulated specifically for hormone absorption), the results are a different story.

The Andrology Department at Concord Hospital and ANZAC Research Institute did a study on testosterone cream applied to scrotal skin. Most importantly, the patient’s symptoms resolved without side effects.

They observed a striking result: testosterone bioavailability through scrotal application was up to EIGHT times higher than other areas of the body. That’s a huge difference for men seeking optimized hormone levels. The key to this effectiveness is using the right base.

The cream also offers a slightly better increase in dihydrotestosterone (DHT) levels; therefore enhancing erectile strength and overall sexual functioning. As DHT is crucial to a man’s libido, it makes sense that the trans-scrotal cream has an edge over intramuscular injections. 

This happens because applying testosterone to the scrotum leads to greater exposure to the enzyme 5α-reductase, which converts testosterone into DHT—a more potent androgen that plays a big role in libido, sexual function, and energy.

Daily application of testosterone cream better mimics your body’s natural testosterone rhythm, helping reduce the hormone spikes and crashes that can happen with injections.

Are There Any Downsides to Testosterone Cream for Men?

The only real downside with the trans-scrotal cream is the possibility of wiping it off on someone else by accident. You will have to avoid certain activities for up to 90 minutes for maximum absorption of testosterone to take place (sex, working out, and showering).

Some men may also find the daily routine of applying the cream less convenient than getting a weekly or bi-weekly injection. However, newer hormone-specific HRT base creams absorb quickly, dry fast, and don’t leave behind an oily residue—making the process much easier to stick to.

Do You Get the Same Results from TRT Creams and Injections?

Aside from these primary differentiators, the creams and injections provide very similar results under the guidance of an experienced physician. You’ll end up getting the same physiological benefits with either method: muscle mass gain, cognitive enhancement, greater energy levels, and endurance.

The key is consistency and proper dosing—both of which can be achieved with either form of testosterone replacement therapy (TRT).

At the end of the day, the best option is the one you’ll actually stick with. Whether it’s cream or injections, both can help balance your testosterone and estrogen when used the right way.
